Our work

Our services, and specialized outpatient palliative care in general, are aimed at palliative patients and their social environment if the intensity or complexity of the problems resulting from the course of the disease make it necessary to deploy a specialized palliative care team - temporarily or permanently. The basis for this is a care structure based on palliative care. This includes, in particular, specialized palliative medical and palliative nursing advice and/or (partial) care, including the coordination of necessary care services through to comprehensive, individual support management. Multi-professionalism, 24-hour availability seven days a week and specialist status (through further training and experience) are essential for us.

What is SAPV?

We are contact persons ...

  • for patients and their relatives
  • for general practitioners
  • for outpatient care services
  • for inpatient care facilities for the elderly
  • for all those who want to enable seriously ill people to remain in their home environment and need support in doing so
How does the SAPV system work?

It starts with the first contact: You can contact the SAPV coordination center directly or contact your family doctor. You will find the telephone number at the bottom of this page.

The coordinator will conduct the initial consultation with you at short notice at home, in hospital or in the facility.

The care provided by a palliative care doctor, a suitable care service, aids, etc. will then be arranged by the coordinator. The care service remains your first point of contact throughout - and is available around the clock.

What does outpatient palliative care involve?

Specialized outpatient palliative care includes

  • Detailed advice and information in the initial consultation
  • Guidance and advice for patients and their relatives on all palliative medical and nursing issues
  • Support in crisis situations
  • Forward-looking emergency planning
  • Medical and nursing support during the course of the illness
  • Support in coping with everyday life, hospice care during the dying phase, farewell and mourning

SAPV is provided regardless of the level of care and any basic care already provided. SAPV focuses on the individual wishes and needs of patients and their relatives. To this end, we work together with a network of GPs, pharmacies, medical supply stores, physiotherapists and other stakeholders.

Who pays the costs?

The costs of specialized outpatient palliative care are covered by the statutory health insurance funds according to the doctor's prescription. Similar guidelines apply to privately insured persons. We will be happy to advise you individually.
