Networked for a cared-for end of life: Together, humane, reliable
People with a serious, life-shortening illness with no prospect of a cure need special care and support. Suddenly, the focus of treatment is no longer on prolonging life, but on maintaining the best possible quality of life through closeness, care and the alleviation of pain and other symptoms. The focus is on the sick person and their relatives and loved ones with their very individual wishes and needs.
In our palliative care network, many dedicated people work hand in hand to support seriously ill and dying people and their relatives and loved ones in a challenging phase of life. Together, we are committed to ensuring that no one is left alone. Quality of life and self-determination are made possible until the end - at home, in a nursing home or other facilities, in hospital or in an inpatient hospice.
People with a progressive and incurable illness require very special individual care and support, which can only be achieved through comprehensive and interprofessional networking and cooperation between all specialists and stakeholders in hospice and palliative care.
Pflege-und Servicezentrum GmbH Neumünster is a joint institution of Friedrich-Ebert-Krankenhaus and Diakonisches Werk Altholstein GmbH. It was founded in 2010 with the aim of establishing and ensuring specialized outpatient palliative care (SAPV) in Neumünster and the surrounding area. To this end, a comprehensive network consisting of registered doctors, care services, care homes, the outpatient hospice service, the hospital, medical supply stores and pharmacies has been formed.
Based on this existing network activity, Pflege-und Service GmbH applied for funding for regional network coordination in accordance with Section 39d SGB V. The network coordinator, Michal Weithoff, actively supports the targeted
joint cooperation of the various regional players in hospice and palliative care. This involves all stakeholders, organizations and services that can contribute to the improvement and further development of hospice and palliative care in the city of Neumünster and the surrounding area.
Our aim is to alleviate suffering, maintain quality of life and enable a dignified life until the end - regardless of gender, age, religion or origin.
